Derek is rendered sleepless by a snoring Meredith, and is forced to tell her after she realises that he has not been sleeping well. She seizes the opportunity to tell him about his bad morning breath, but finds out that he has been stealing away every night to sleep elsewhere and creeping into bed before she wakes up.

Izzie is assigned to prepare Heather Douglas, a cynical and bitter teenager with severe scoliosis, and a past medical history of VATER syndrome. Heather is in to have kidney stones removed to improve her quality of life, and Callie feels that she is suitable for a spinal surgery that will straighten the curvature and enable her to stand straight. However, a consultation was not requested for, so Callie seeks advice from Bailey on how to get Derek to perform the consulatation. Callie successfully manages to stare Derek down and forces him to perform the consult. Izzie wants in on this rare surgery, but Bailey refuses on the grounds that her psychiatrist has yet to clear her for surgery because she has not deposited her 8.7 million dollar check. After being pressured into doing so by George and Bailey, she eventually deposits the check, but not before breaking down at the bank. However, the surgery cannot proceed as the insurance company deems the surgery too risky and experimental, and Heather's mother cannot afford the otherwise expensive surgery.

Mr O'Malley decides to let the Chief and Bailey remove the entire tumor even if the cancer has spread, despite their medical advice. They reluctantly agree to the private request, and resolve not to tell George of this arrangement, even though George has requested to be told the truth about his father's condition. During the surgery, the cancer is found to have spread but the surgeons proceed anyway. Meredith, who is also in the operating room, is sworn to secrecy. Meanwhile, George approaches Burke to better understand his father's condition. Delighted at his father's supposedly successful surgery, he kisses Callie in joy, surprising both her and his family.

Despite not being assigned to her niece's jejunal atresia surgery, Meredith repeatedly bumps into her father Thatcher, who is visiting his granddaughter. She avoids him every time as he unsuccessfully tries to approach her.

Mark is in a foul mood as he fails to adapt well to Seattle and Addison's rejection. Alex bears the brunt of his temper as Addison tries to intercede on his behalf, admonishing him for abusing his interns. Things seem to deteriorate as Alex fetches Mark his hated beverage, and also decides that plastics is not for him. As Alex and Addison bond over Meredith's niece bowel perforation complications, he reveals that he deliberately gave Mark the wrong coffee because of how shabbily Mark was treating her. They almost kiss, but get interrupted when a nurse barges into the room, causing them to part on awkward terms.

Cristina and Burke refuse to speak to each other as they are too proud to give in, causing Cristina to seek all sorts of avenues to find out about Burke's hand without asking directly. She gets everyone from Meredith, Derek, George and even Olivia to pry, but fails.

Meredith tells Derek that she has abandonment issues, and wants Derek in the same bed as her every night. Derek agrees, but is once again plagued by her incessant snoring.

The episode was originally intended to be one episode, but was later expanded to a two-parter.
The song that closes the episode, "Love Will Come Through" by Travis was originally written for Moonlight Mile, a film that Ellen Pompeo appeared in. In an odd coincidence, the movie's title (like all Grey's Anatomy episode titles) come from a song, in this case a song by the Rolling Stones.
This is the first episode to feature the closing credits "To Be Continued" instead of the normal "Grey's Anatomy".

After a successful operation on his heart, George's father undergoes surgery for his cancer, Thatcher Grey visits Seattle Grace in order to see his new granddaughter, and Meredith discovers that Derek has trouble sleeping soundly. George's father dies. Addison reveals that she aborted Mark's baby. Part two and conclusion of a two part episode arc.
